  • Mail  request to schedule a Appointment:  If you are paying by credit card please include your phone number, address, credit card number, expiration date and your name as it appears on your credit card. If you are paying by check include phone number. You may also pay with a money order. Lily will contact you by phone to schedule your appoint-  ment as as soon as your method of payment clears.
